JOB DESCRIPTION SUMMARY
The registered nurse plans, organizes and directs home based care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care
- Completes the initial and ongoing comprehensive assessment to understand patient needs and to set goals and interventions
- Initiates the plan of care and makes necessary revisions as needed
- Provides professional nursing care by utilizing all elements of the nursing process
- Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and independence
- Reports changes in the patient’s condition
Communication
- Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given
- Communicates with the team regarding the patient’s needs
- Recognizes and utilizes opportunities for health counseling with patients and families/caregivers
- Attends case conference meetings
Additional Duties
- Obtains/receives physicians’ orders as required
- Participates in on-call duties as defined by the on-call policy
- Desire for personal growth and development
- Fulfills the obligation of requested and/or accepted case assignments
- Actively participates in quality assessment performance improvement teams and activities
POSITION QUALIFICATIONS
- Current SD Nursing License with minimum of 2 years in acute care setting or equivalent
- Valid driver’s license
- Automobile Liability Insurance
- Ability to pass background and drug screening
- CPR certification, preferred
- Strong communication and interpersonal skills
- Teamwork orientated
- Ability to multi-task and self-motivated
- Understanding of palliative approach to care
