Physical Therapist: PRN

Job Description:

  • Performs PT assessments, diagnostic tests, skilled procedures and ongoing evaluations of patients following current Plan of Care.
  • Complete Assessments and OASIS within 48 hours of referral unless otherwise ordered.
  • Develop Therapy Plan of Care and give direction to other team members in regard to patient needs, findings, POC and interventions per Agency Policy. Attends all meetings, Case Conferencing and required educational programs. Participates in discharge planning.
  • Communicates with PCP, Clinical Manager and other team members as condition changes for best practice and seamless continuity of care regarding patient needs and progress.
  • Implements teaching appropriate to patient needs, environment, knowledge level and learning ability.
  • Plans, teaches, supervises, counsels and supports patients to promote improvement, recovery and symptom management.
  • Uses OASIS assessment tools to improve quality of care and patient outcomes.
  • Practices Infection Prevention and Control as outlined in Agency Policies, demonstrates knowledge of Infection Control Policies, recognizes and responds to unsafe situations and reports this to Management. Demonstrate safe practices in use of all equipment to prevent accidents and promote safety.
  • Perform in home supervision of Home Health Aides as directed by Agency Policy.
  • Performs other job duties as delegated within PT Scope of Practice, per Agency Policies and in accordance with all state, federal regulations and standards.
  • Accepts responsibility for personal and professional development and identifies learning/developmental needs to Supervisor.

Qualifications

  • Current license/registration as Physical Therapist in South Dakota, Minimum of 2 years’ experience in health care field with home care experience preferred. Current CPR Certification, strong verbal and written communication as well as good interpersonal skill set. The ability to work independently with minimal supervision. Licensed driver with automobile insurance and good driving record, ability to pass a background check.
